AutoTotemV2 is a Fabric client mod that automatically moves a totem of undying into the off-hand when a player's stock runs low.

What it does

AutoTotemV2 keeps a totem of undying ready in the off-hand so a player is protected from lethal damage without manual swapping. The mod runs on the client alone, so only the player who installs it needs it and servers do not. Builds exist for 1.21.4 – 1.21.8, with 1.0.0 for 1.21.8 as the latest file, released under the Apache License 2.0 and open source.
