DeathHistoryNotes
by thVAZIK
DeathHistoryNotes records every player death in a Minecraft world and keeps the details in a notes interface that players can open at any time.
What it does
When a player dies, the mod writes an entry that holds the event and its details, including the location and the time it happened. The entries accumulate over the course of a world, so the history of deaths stays available instead of disappearing with the respawn. Players open the Notes interface to browse the full death history.
The mod is an add-on to the Notes mod and needs it to run, so Notes must be installed alongside it. Fabric API is also required. DeathHistoryNotes runs on the client only, which means only the player who wants the history needs the mod and the server can run without it. Files are published for Fabric, for 1.21.5 – 1.21.10, with 1.0 for 1.21.10 as the latest build, and the code is open source under the MIT License.
Key features
Automatic death records
Each death is logged without any action from the player, so no death goes unrecorded while the mod is installed.
Location and time details
Every entry stores where and when the death occurred, which makes it easier to return to the spot or match an event to a play session.
Notes interface view
The death history opens in the Notes interface, where entries can be read in detail.
Add-on to Notes
The mod extends the Notes mod, so its entries sit alongside the other notes a player keeps.

Use cases
Revisiting a fatal spot
A player who lost items in a dangerous area can check the stored location to find the place again.
Reviewing a long session
Anyone reviewing a long world can scroll through the full list of deaths and their times.
Who it’s for: Players who want a record of their deaths in survival worlds, and players who already use the Notes mod and want to track deaths alongside their other notes.
How it compares
InvDeath Mod is a Fabric mod for 1.21.1 that runs on the server only, and it is released under the MIT License, but it is not open source. Death Count is a Fabric mod for 1.21.1 – 1.21.11 that also runs on the server only, and it is open source under the MIT License. Play Time Tracker is server-side only and open source under the Apache License 2.0, for Fabric 1.21 only. DeathHistoryNotes, by contrast, runs on the client and is built as an add-on to a separate notes mod.
