Rifts
by SIDEREAL
Rifts adds physical portal blocks that teleport entities, including items and mobs, between linked locations within the same dimension.
What it does
A rift is built from obsidian and an end crystal, placed on an obsidian pattern with crying obsidian three blocks above and a two-block gap. The rift is linked to a lodestone through a compass, and the compass is consumed when the link is made. Stepping into or dropping an item into a rift sends it to the linked lodestone location. Rifts can warp any entity, not only players, and links persist if a lodestone is removed later, so a rift can keep teleporting after its target is gone.
Rifts is a server required mod, with client installation optional. The server must run it, and players may add it without any server-side setup. Builds are published for Fabric, Forge, NeoForge and Quilt, covering 1.20.5 – 26.1.2, and a data pack version is also available. The code is released under the MIT License and is not open source.
Key features
Physical rifts
Rifts are placed blocks that move entities through the world, so travel becomes a fixed structure rather than a command.
Lodestone linking
A rift is tied to a lodestone with a compass, and that link decides where entities arrive.
Entity and item transport
Mobs, players and dropped items entering a rift are carried to the linked location.
Same-dimension travel
Rifts only connect locations within the same dimension, so they do not cross into the Nether or the End.

Use cases
Base transport
A builder can connect a remote farm or storage room to a central base with a pair of rifts and a lodestone.
Automated item routing
Dropping items into a rift moves them to a linked point, which can feed a collection area without hoppers.
Who it’s for: Players and server owners who want fixed teleport points inside a single dimension, and builders who want item and mob transport built from world blocks.
How it compares
Lodestone Warps is server required with client optional, under the Creative Commons Attribution Non Commercial Share Alike 4.0 license, not open source, and covers 1.20 – 26.2. Warp Totem runs on client and server, is open source under the GNU General Public License v3.0 only, and covers 1.19 – 1.20.4 on Fabric and Forge. Rifts is not open source and differs by using placed blocks rather than commands.

What the author says
Rifts introduces physical rifts in your world which teleports all entities to the linked location. The rift is made with obsidian and an end crystal, and must be linked to a lodestone through a compass. You can have as many linked rifts as you want, and they keep on teleporting after the lodestone is removed.
