Kiwe's Undertale Death Screen replaces the usual death screen with an Undertale-style one, built around an animated heart that shatters when the last heart is lost.

What it does

On death, the player sees a custom Undertale-style screen with an animated heart. The heart skin changes with status effects: poison, wither and freezing each give their own look. When the last heart is lost, the heart shatters into flying pieces, as in the game. A determination track plays during the screen, and hardcore mode is supported.

The mod is a fork of the original Undertale Death Screen, with the concept and base code kept from the original author and the work updated for Minecraft 1.21.11 on Fabric. It is actively maintained by the current maintainer, and Cloth Config integration allows the screen to be customized. Credits are shown as part of the screen.

The mod is published for Fabric and Quilt, with 1.3.3 for 1.21.11 as the latest file, dated 2026-07-14. Only the client needs it, so servers run without it. It is released under the MIT License and is not open source.
